The City’s Annual Wellness Assessment (AWA) is now open and runs until July 31, 2026. The program is designed to support and improve employee health and wellness. Completing the AWA requirements will make you eligible for a health insurance premium discount and provide valuable health insights. 

How To Earn Your Premium Discount

To qualify for a discount of up to $100 per month starting January 1, 2027, you must complete the following components by July 31, 2026:

Total Possible Premium Discount $100

All three components must be completed by each employee, retiree, and covered spouse (if applicable) to receive the full discount. Components may be completed in any order. If you have a covered spouse, both of you must complete all required components to qualify. Employees hired on or after June 1, 2026, are exempt from needing to complete this year's AWA.

Registering for WebMD ONE Portal

To get started, employees and covered spouses must individually create a WebMD ONE account or download the Wellness at Your Side app with connection code “Raleigh". Additional information is provided online.

If your account has already been created, log in using your existing username and password. If your password needs to be reset, please contact WebMD Customer Service at 888-932-6312 or go through their Account Recovery process online.

Biometric Screening

To complete the biometric screening, you have two options:

  1. Healthcare Provider: If you completed a physical with your healthcare provider between Aug. 9, 2025, and July 31, 2026, download the 2026 Healthcare Provider Form, and have your provider complete it. Once the form is completed with the required metrics and signatures, you may upload it. Alternatively, the form may be faxed or mailed to the number or address listed at the bottom of the form.
  2. City-Sponsored On-Site Screenings: The City has partnered with WakeMed to offer on-site biometric screenings from April to July 2026. These screenings will be held at various times and locations to accommodate the maximum number of participants, and can be registered for online. WakeMed will automatically upload the data collected at these screenings to the WebMD ONE portal.

If you are pregnant or on leave, you can contact wellness@raleighnc.gov before the July 31 deadline to request a potential exemption from the screening. You will still need to complete the health risk assessment and a wellness activity.

Health Risk Assessment (HRA)

This is a questionnaire that identifies potential health risks based on your lifestyle, medical history, and environmental factors. Completing a health risk assessment is important for gaining insights into one's personal health status, identifying potential risk factors, and proactively addressing lifestyle choices that may impact well-being. To complete this requirement, you may access the assessment in your WebMD ONE  portal. It is accessible on your homepage or by clicking on the menu option.   

Additional Wellness Activity Participation

You can complete one of the following:

  • City-sponsored virtual or on-site wellness event;
  • Age-appropriate preventative screening
    • Examples:  Dental cleaning, vision, exam, vaccinations, mammogram, prostate screening, cervical cancer screening, colorectal screening, or vascular screening
    • Must have been completed between Aug. 9, 2025, and July 31, 2026
  • Counseling or coaching
    • Examples:  Nutrition counseling, health coaching or mental health counseling
  • WebMD ONE portal activity
    • Examples:  Log 600 minutes of exercise, 150,000 steps, or track sleep for 21 days, complete a wellness challenge, or complete a daily habits plan

Activity participation may be automatically reported through healthcare claims, or self-reported on your “Rewards” page in the WebMD ONE portal. 

AWA Information Sessions

Each AWA Information Session will cover the AWA requirements, including biometric screenings, the health risk assessment, and the wellness activity. Discover how completing these components can earn you a health insurance premium discount and gain helpful insights to guide you through the process.
