About the AWA Participation Biometric Screenings Health Risk Assessment and Wellness Activities WebMD ONE Portal

The City’s Annual Wellness Assessment (AWA) is now open and runs until July 31, 2026. The program is designed to support and improve employee health and wellness. Completing the AWA requirements will make you eligible for a health insurance premium discount and provide valuable health insights. 

Here you will find answers to your Frequently Asked Questions (FAQs) about the AWA. About the AWA

What is the Annual Wellness Assessment (AWA)?
The AWA is designed to promote healthy lifestyle behaviors among employees, retirees, and covered spouses. Participation in the AWA program also provides a premium discount. 

What are the three components of the AWA? 
The three components are biometric screening, health risk assessment, and additional wellness activity.

Once I have completed all three components of the AWA, how do I know that I am receiving the health insurance discount?
You can confirm completion for the AWA by logging in to your WebMD ONE account, clicking on the “My Health Tab” at the top center of the page, scrolling down, and clicking the “Rewards” tile. This is where you can view the completion status for each AWA component: health risk assessment, biometric screening, and one wellness activity. 

Participation

Will I lose medical coverage if I do not participate in the AWA?
You will NOT lose medical coverage if you don't participate in the AWA. Participation in this program is voluntary; however, if you do not participate, you will not qualify for the health insurance premium discount for the 2027 plan year.

My spouse is covered under my medical plan. Is my spouse required to complete all three steps (biometric screening, health risk assessment, and one wellness activity) individually?
Yes, if your spouse is currently covered under your City of Raleigh medical plan, both you and your spouse must complete the AWA individually. Each component must be completed by July 31, 2026, to be considered compliant and to receive the health insurance premium discount for the 2027 plan year. 

I am a new hire with the City - do I need to complete the AWA?
If you were hired after May 31, 2026, you do not need to complete the AWA for the 2027 medical plan discount; however, you will need to participate during the 2027 calendar year and beyond to receive the discount.

Biometric Screenings

How do I complete my biometric screening?
Eligible participants have two options to complete their biometric screening. Individuals who are pregnant or on leave may qualify for an exception to the biometric screening requirement and may contact Human Resources in advance of the deadline to confirm their exemption.

  • Attend a City-sponsored on-site biometric screening: Screenings will be held between April 13 and July 23, 2026 (appointment link coming soon). Appointments are required - only confirmed appointments can be guaranteed, while walk-ins will be waitlisted. WakeMed will collect and upload your on-site biometric data to the WebMD ONE Portal on your behalf.
  • Complete a biometric screening form with your healthcare provider:  If you complete a physical or biometric screening between Aug. 9, 2025, and July 31, 2026, you may print the 2026 Healthcare Provider Form and have your provider complete it. All values with an asterisk (*) are required. Both you and the provider must sign the form. Once completed, you have three methods for submitting your completed biometric screening form. You can expect a confirmation email from WebMD ONE within 48 hours of submitting your form, and your biometric data should be in your WebMD account after 10 business days. If you don't receive an email confirmation within 48 hours of submission, please resubmit. Forms received after the July 31 deadline will not be accepted.
    • WebMD ONE Portal Upload (Preferred):  Visit the WebMD ONE portal, click “My Benefits," then click the “Upload Your Health Care Provider Form” Tile. Completed forms may also be uploaded to the WebMD Provider Form Portal.
    • Fax: Send your completed form to 402-939-0604. Please note that your provider cannot fax the form on your behalf; you must submit it yourself. It is recommended that you keep a copy of your fax receipt for your records.
    • Postal Mail:  Send your completed form via postal mail to Total Wellness, Attn: Data Team, 9320 H Court, Omaha, NE 68127. Please allow sufficient time for mailing. 

If I know my biometric data and enter it myself when responding to Health Risk Assessment questions, do I still need to complete a biometric screening, or have I met the biometric screening requirement?
You must complete the biometric screening using one of the two options outlined above: attending a City-sponsored on-site screening or completing a biometric screening form with your healthcare provider.

I registered for a City-sponsored on-site biometric screening, but can no longer attend. What should I do?
If you are unable to attend your scheduled on-site biometric screening, you must cancel your appointment and select a different date using the biometric screening scheduling link (coming soon).

Health Risk Assessment and Wellness Activities

How do I complete my online Health Risk Assessment?
Log in to the WebMD ONE Portal and click "Take the Health Risk Assessment". Please note that if you are new to the City or if this is your first time participating in the AWA, you will need to create an account and complete the enrollment information. You can also request a username or password reset on this login page. The WebMD ONE wellness portal is available year-round to access step challenges, well-being trackers, wellness resources, and more, including the health risk assessment!

I am completing my online Health Risk Assessment, but my biometric screening has not yet been conducted. What should I do when I am asked for my biometric data?
When completing your online Health Risk Assessment, you may include your most recent metrics or skip the question. When you do complete your biometric screening, WebMD will process the most recent data on your behalf.

I completed my online Health Risk Assessment - how do I see my results?
You can view your results at any time by logging in to your WebMD profile, then clicking the menu bar and scrolling down to “Health Risk Assessment." There, you will be able to view or print your report. You may also review tips for staying on track with your wellness goals. 

I completed my online Health Risk Assessment, and now the website is recommending programs based on my data. What are these programs?
Participating in these digital health promotion programs year-round can help improve your overall health and well-being. These programs are voluntary and available to you at no cost. They include various topics such as activity trackers, nutrition guides, stress-reducing activities, and personalized health goals to keep you motivated. You can access these programs online through the WebMD ONE portal or download the free WebMD app. 

I attended a wellness activity - how do I report participation?
Activity participation may be automatically reported through healthcare claims. If you attend a City-sponsored wellness activity, you must self-report it on your “Rewards” page in the WebMD ONE portal.

I recently retired from the City - how do I access my WebMD ONE account?
If you recently retired from the City and used a City of Raleigh email address for your WebMD One account, you will need to call WebMD Customer Service at (888) 932-6312 to reset your account and regain access. 

My spouse is not a City of Raleigh employee. How do they create a WebMD ONE account?
Spouses will create an account and log in to the same portal. Your spouse's registration ID will be your employee ID + the spouse’s date of birth + the spouse's full first name. For example, if your employee ID is 012345, the spouse's birthdate is 09/01/1985, and the spouse's full first name is Joseph, the registration ID would be: 01234509011985Joseph. Once registered, use the same credentials to log back into the account.

I am having trouble logging into the WebMD ONE portal. Who should I contact for technical support?
