Care Navigation Team

A team of case management and peer support specialists

After an initial crisis response, the Care Navigation Unit provides follow-up support and stability for community members.  Read how Raleigh CARES is already making a difference.

What is Care Navigation?

The Care Navigation team connects with community members in crisis who are experiencing homelessness after their initial contact with a first response team.  

The team will work to identify specific housing needs and match the person with available community resources. This team will also better connect the various City services needed to support the person. This effort aims to ensure no one falls between the cracks.  

Pilot Scope

The Care Navigation Team is focused on people experiencing homelessness and housing instability. The team, comprised of three members, has capacity for 30 active cases at a given time.

The team operates within standard business hours: Monday-Friday, 9 a.m. to 5 p.m.
