Care Navigation One Year Update

What is Care Navigation?

The Care Navigation team connects with community members in crisis who are experiencing homelessness after their initial contact with a first response team. The team works to identify specific housing needs and match the person with available community resources. This team also connects the various City services needed to support the person. These efforts aim to ensure no one falls between the cracks.

Care Navigation One Year Update

In its first year, Care Navigation quickly became a nimble, effective team dedicated to preventing homelessness, stabilizing households, and connecting residents to long-term support.
