The forms needed to request the disclosure of video according to NCGS §132-1.4A are available at the links below.
Both pages of the completed form may be delivered in person to any Raleigh Police Department district station, sent by email at internal.affairs@raleighnc.gov or delivered by U.S. Mail to the Office of Professional Standards (mailing address is located in the contact section).
Once the forms are received, they will be reviewed within three business days and the person that filled out the form will then be contacted.
