Online Form Downloadable Paper Form Claims Process
If you have been involved in an accident and/or wish to file a claim against the City, you must submit a claim form.
Please provide as much information as possible and make sure to attach any material(s) you may have, such as:
- a police report,
- pictures of the damages/injury, an estimate of damages,
- and any other documentation you deem necessary.
Downloadable Paper Form
If you are unable to complete the process using the online form above, a downloadable paper form is also available. This form can be mailed or emailed to Risk Management at the address listed below.
Upon receipt of the claim, a claims adjuster from the City’s third-party administrator will contact you within 48 business hours. An investigation must be conducted by the claims adjuster before a final decision on liability and/or payment(s) will be made. There are many variables during the claim investigation, and this process can take up to 30 days or longer depending on the nature of your claim.
